How to Complete Consent form

Applicant can apply for GCMS files through the Access to Information and Personal Information Request by giving us your written Consent to your application. In the form you have to mention your basic details along with application no you wish to request GCMS notes or Caips notes and sign it with blue ink and forward it us.

How i can request GCMS notes direct from IRCC

Applicant can apply for GCMS files through the Access to Information and Personal Information Request. They can download the form from direct link. If they want to send the form, they must be a Canadian Citizen or Permanent Resident of Canada. If applicant is not a Canadian or a Permanent Resident of Canada, they can find someone in relation who is living in Canada. A Canadian who is living there they can also file the Access to Information and Personal Information Request form from outside Canada, but fees will be apply in that case.
Remember that if someone files more than one request on behalf of applicant, then Applicant must have to sign a separate consent form for each individual request.
The the applicant will need to write a Formal Request letter addressed to IRCC where they actually request the GCMS notes or the notes from the older systems.
